  • Curriculum vitae including education, training, and experience in composing music for dance, opera or music theatre
  • PDF score of a dance, opera or music theatre work that received professional premiere performance within the last three calendar years
  • URL link to recording or video on YouTube or Vimeo matching the submitted score
  • Statement describing mid-career profile and how the Award would advance the applicant's career, up to 750 words
  • Short biography and photo for publicity purposes if selected as winner, up to 250 words
