• Brief description of yourself and your top three proudest writing achievements (in lieu of CV)
• 3-page sample of your writing, preferably from the story you intend to read, up to 3 words
• 1–2 sentence pitch of the story you will read (if not submitting a sample from that story), up to 2 words
• Indicate if this will be your first reading and, if not, where you have read before
• Confirmation that you understand this is an in-person event and can attend on October 27, 2026
• Website or social media link (optional)
